到家計會做墮胎手術,家庭醫生簽署可以嗎?是否要家長簽署同意的?大概要多少錢的?

本人十七歲，我懷了孕，若到家計會做墮胎手術，是要兩個註冊醫生簽署的，(1) 我想問可否是普通的家庭醫生？(2) 做這個墮胎手術，是否要家長簽署同意的？(3) 做這個墮胎手術，大概要多少錢的?
