腰痛放射到大腿前部

本人60歲，突發腰痛一月餘，牽扯大腿前部，看過幾次醫生西醫只給止痛藥（靠止痛藥才能睡覺）；中醫敷藥可止痛但又皮膚過敏，無法繼續治療，看了網上的介紹，想找葉醫生治療請問如何預約？費用多少？我最近為此病做過體檢和X光需要帶上嗎？可以預約9月4號下午嗎？謝謝！
