牙套是否一定要裝牙釘

醫生您好
因為本人前臼齒被蛀了1/2隻,加上8年前曾於內地正規醫院杜牙根,但很久很久便會痛一次.
牙齒的蛀面大,令牙齒的填充物填落,日前曾看牙醫,影了x光,醫生建議為先行補好牙作觀察半年,觀察是否因為杜牙根的關係而令牙齒疼痛腫大,之後再為我打牙釘做牙套,唯套牙套的家人認為不用打牙釘.

請問醫生
1.以我的情況來看,是否要打牙釘?及打牙釘有何風險?
2.我的牙醫做法洽當嗎?
