Obesity can be due to metabolic / endocrine problems, but the management is mainly through diet, exercise and lifestyle changes. Full assessment with a doctor, perhaps a family doctor first may be a good start.
Besides, hHigh BMI is associated with a myriad of female reproductive health problems. Childhood obesity is associated with early onset of puberty, menstrual irregularities during adolescence and polycystic ovary syndrome. Women of reproductive age with high BMIs have a higher risk of ovulatory problems and tend to respond poorly to fertility treatment.

It depends on which kind of contraceptive injectables – monthly or 3-monthly? Menses can be irregular with the 3 monthly injectables. Assuming if you are having regular monthly injectables, the breakthrough /irregular bleeding may be abnormal- time to check up. If there is any concern, it would be worthwhile to go for a thorough consult and assessment.
