The information provided suggests probable lower urinary tract symptoms related to the prostate that do not respond well to medication for the prostate and urinary tract infections. More in depth investigation is needed to arrive at a definitive diagnosis before proper treatment can be administered.

The painful swelling of acute bacterial epididymitis usually responds well to antibiotics except in those with repeated attacks when chronicity has set in. Addition consideration is other underlying pathology masquerading as infection. Do revisit the specialist to ensure nothing goes amiss.
Any pigmented lesion protruding from the genitalia needs early attention and proper treatment.

Possible meatitis
病患者男 - 26歲
Hello,
I’m not sure if I have meatitis but meatus does not look normal. Meatus would poke out like lips when I wear tighter pants. I do not have STD and previous biopsy confirmed that I do not have Lichen Sclerosus. Although this condition is not causing any urological problems, it is impacting my life significantly because I cannot sit comfortably. I’m currently using neosporin but that does not look to help much. What would be the remedy for that? Surgical intervention? Thank you!

"Meatus would poke out like lips when I wear tighter pants." does not sound like meatitis in the absence of lower urinary tract symptoms or signs. To arrive at a definitive diagnosis, consulting a urologist is the best option.
The condition described is recurrent urinary tract infection despite treatment. More in-depth investigations are indicated. A urologist is in the best position to do so.
