指甲有濃

病患者男 - 6O歲

本人指甲長期裂開，指甲不會向前生長，而且甲型變得灰黑，但又不是灰甲，指甲只會向上生長，指甲厚，現在指甲有濃，以及有臭味，已服兩星期抗生素，仍然有濃!請問醫生如何處理!
