去廁所會痛

病患者男 - 22歲

數天前包皮開始痕癢，使用白花油後能暫時援解但過兩天後不再痕癢，轉為龜頭位置開始痛。服用mefenamic acid 250mg 及使用含有miconazole 軟膏後痛楚減輕,但排便時痛楚頗為強勁甚至加重。　

請問是什麼病痛，可以服用成藥治療嗎？　或是一定要看專科／內外全科醫師才可以治癒？　謝謝
